Baptism Preparation Classes for parents and godparents are held in the Conference Room of the Assumption House on the 3rd Thursday of the month at 6:30 pm. Both parents and sponsors must attend.
Check the Parish Bulletin for specific times and locations of these classes.
Call the parish office for an appointment to meet with the pastor to schedule the baptism.
Baptismal Application Packet
An official copy of the child’s birth certificate.
If parents are not registered members of St. Mary of the Assumption: a letter of permission for baptism from home parish.
A Sponsor Covenant Form for each sponsor.
When a child or an adult is to be baptized, he or she must have at least one godparent. Church Law requires that a child presented for Baptism, that is, for membership in the Church, be given a godparent who is willing and able to help the child develop and grow in the Catholic faith.
Although it is customary for children to have two godparents only one is required. When there are two, one must be male and the other female.
Godparents must meet all of the following criteria, which are established by universal Church law and which do not vary from place to place.
Have completed the 16th year.
Be a practicing Catholic who has been confirmed and has already received the sacrament of the Eucharist and leads a life in harmony with the faith and the role to be undertaken.
If married, is in a valid Catholic marriage.
Not bound by any canonical penalty.
Not be the mother or the father of the one to be baptized.
Complete the Baptism Sponsor Covenant form.
